If you are a broker licensed with the Virginia Bureau of Insurance and would like to become appointed to do business with Delta Dental of Virginia, it’s easy to get started. Simply complete our Agent/Agency Data Request (ADR) Form and mail, fax or email a copy to:

Once we receive your ADR form, we will forward an appointment package to you for completion. Upon your completion of this package, you will become an appointed Delta Dental of Virginia broker and can begin creating quotes and selling business to your clients!
